The purpose of this practice-based thesis was to create a feedback survey form for a professional family home that is the commissioner of this thesis. The purpose of the feedback survey is to help the family home develop its operation, and if necessary, improve the procedures. Family care refers to alternative care provided 24/7 in a foster family or family home in accordance with the law on supervision of private social services. Professional family homes are obliged to provide good quality social care and provide good treatment. The client’s wishes and opinions must be considered individually. The theoretical background deals with child welfare, inclusion, and the best interest of the child. Thus, the key concepts were child protection foster care, professional family home, inclusion, and the best interest of the child. In addition to the theoretical background, the company's team meetings, which were used in the content construction of the form, served as an aid in creating the questionnaire. The questionnaire was tested by a test group. The data were collected electronically using a Webropol survey. Dividing the question types into themes outlined how the respondent experienced the form and answered the questions asked. According to the information received from the test respondents, the true/false questions in the feedback form were easy to answer. On the other hand, open-ended questions were not so popular. Everyone who answered the form answered all the true/false questions. Out of eight who received the survey form, five people answered.
