Toiminnallisena opinnäytetyönä teimme potilasoppaan Ylä-Savon SOTE kuntayhtymä Iisalmen sairaalan dialyysiosastolle. Osaston hoitohenkilökunnalla oli tarve saada osastolle oma potilasopas potilasohjauksen tueksi. Potilasoppaaseen on koottu keskeisimmät asiat aikuisen hemodialyysipotilaan hoidosta. Tavoitteenamme oli laatia potilasopas helpottamaan pitkäaikaissairauden kanssa elämistä sekä motivoimaan omahoitoon. Potilasopas palvelee yksikön sairaanhoitajia potilasohjaustilanteissa sekä nostaa osaston imagoa. Potilasopasta käytetään potilasohjauksessa sekä opiskelijoiden ja uusien työntekijöiden perehdytyksessä. Potilasohjauksen onnistumisen esteenä voi olla liiallinen tiedon saanti. Suullisesti saatu ohjaus unohtuu helposti ja keskeinen asia voi jäädä huomioimatta. Suullisen ohjauksen lisänä annettava kirjallinen materiaali auttaa potilasta sisäistämään saamansa ohjauksen. Potilasoppaassa on huomioitu myös omaiset.

Opinnäytetyön tuotos on kierrekantinen potilasopas. Potilasopas on saatavana myös sähköisessä muodossa. Potilasoppaan tavoitteena on antaa potilaalle tietoa ja tukea sekä parantaa hoitoon sitoutumista. Toteutimme potilasoppaan työstämisen teoreettisen viitekehyksen pohjalta. Kokosimme potilasoppaasta pilottiversion, jonka pohjalta teimme kvalitatiivisen kyselytutkimuksen parantaaksemme opasta. Tutkimuksen myötä saimme potilasoppaasta kohderyhmää palvelevan. Kyselyyn vastasivat yksikön potilaat, heidän omaisensa sekä yksikön sairaanhoitajat.

Jatkotutkimusaiheena voisi tutkia Iisalmen dialyysisosaston potilasohjauksen laatua ja päivittää opasta tutkimuksen pohjalta.

The purpose of this functional thesis was to produce guidebook for the Upper Savo Federation of Municipalities in Social and Health Care, Iisalmi hospital dialysis ward. Nurses had a need for the patient guidebook, designed specially on their ward, for support patient guidance. The patient guidebook has collected together all the main things that are involved in adult hemodialysis care. The aim of the study was to create a patient guidebook to help patients live with long-term disease and also motivate self-care. Another aim was also to create the output, which serves the unit nurses in patient counseling situations, and to raise the department's image. The patient guidebook is used for patient counseling, students and new staff orientation. The success of patient counseling can be hampered by excessive information. Verbally patient counseling is easily forgotten and the key issue may remain unnoticed. Oral guidance given in addition to written material helps patients internalize received counseling. In the patients guidebook relatives are also noted.

As a result of the study we created a patient guidebook. The guidebook is also available in electronic form. The patient guidebook is intended to provide information and support to patients and improve adherence to treatment. The patient guidebook has been compiled by using the theoretical framework. We gathered a pilot version of the Patient Guide, on the basis of which we made a qualitative questionnaire survey in order to improve the guide. From the help of the research we were able to improve the patient guidebook to serve the target group even better. Our questionnaire was answered by the ward patients, their relatives and nurses in the unit.

For a further study, the Iisalmi dialysis ward patient guidance could be examined and the quality of the patient guidebook upgraded as a result of the study.
