Teimme opinnäytetyönämme tyypin 2 diabetesta käsittelevän ohjauskansion palvelutalo Abeliin Pieksämäelle. Kansion tarkoitus on koota yhteen ikääntyneen tyypin 2 diabeetikon hoitoon ja ohjaukseen liittyvä keskeinen tieto. Tavoitteena on tiedonsaannin helpottuminen, tiedon monipuolisuus ja sovellettavuuden paraneminen. Tätä kautta voidaan olettaa opinnäytetyöllämme olevan vaikutusta myös ikääntyneiden tyypin 2 diabeetikoiden hoitoon ja ohjaukseen palvelutalo Abelissa. Teoreettisena viitekehyksenä on tyypin 2 diabetes, riskitekijät, ennaltaehkäisy ja elintapamuutosten merkitys sekä ikääntyvän ohjaaminen ja omahoidon motivaation tukeminen. Tyypin 2 diabeteksen hoidon kannalta on ensiarvoisen tärkeää kartoittaa riskitekijät ja ennalta ehkäistä sairaus vaikuttamalla elintapamuutoksien kautta. Jotta tyypin 2 diabeteksen hoito ikääntyneen kohdalla onnistuu mahdollisimman hyvin, täytyy terveysalan ammattilaisilla olla tietotaitoa ohjata ja motivoida diabeetikkoa omahoitoon huomioiden ikääntymisen tuomat erityispiirteet. Kansion työstäminen lähti liikkeelle siitä, että palvelutalon henkilöstöllä oli tarve saada tiivistetysti koottuna tietoa ikääntyneen tyypin 2 diabeetikon hoidosta. Yhteistyössä henkilöstön kanssa pyrimme kehittämään mahdollisimman hyvin tätä tarvetta palvelevan ohjauskansion. Henkilöstö tavattiin kansion työstämisen eri vaiheissa. Aineistoa hankittiin prosessin edetessä. Tukena kansion työstämisessä olivat myös diabeteshoitajat Mikkelissä ja Pieksämäellä, kirjaston henkilökunta ja ylilääkäri Pirjo Ilanne- Parikka, joka auttoi kansion sisällön työstämiseen käytettävien lähteiden valinnassa. Kansion ensimmäisen version valmistuessa sen toimivuutta ja oikeellisuutta arvioi palvelutalon henkilöstö sekä diabeteshoitaja Taru Luukkola Pieksämäeltä. Valmis ohjauskansio luovutettiin palvelutalon henkilöstölle marraskuussa 2011. Raportissa olemme kuvailleet ohjauskansioon kehittämisprosessin etenemistä ja arvioineet kriittisesti sen onnistumista. Kansion työstämisessä yhteistyö palvelutalon henkilöstön kanssa ei sujunut suunnitellulla tavalla, joten henkilöstön rooli jäi pienemmäksi kuin mitä olimme suunnitelleet. Olemme kuitenkin kokeneet ohjauskansion kehittämisprosessin monella tapaa rikastuttavana tehtävänä ja voimme varmasti hyödyntää tulevaisuudessa opinnäytetyömme avulla hankkimaamme tietotaitoamme tyypin 2 diabeteksen hoitoon ja ikääntyvän ohjaamiseen liittyen.

The thesis produced a development folder about type 2 diabetes for an assistant living facility Abeli in Pieksämäki. The development folder was used to offer the personnel of Abeli information about the treatment, symptoms, prevention and guidance about type 2 diabetes of the elderly. The goal was to help the staff find information that was versatile and easy to apply to their work. Therefore, it is presumable that the thesis affects to the treatment and guidance given in the assistant living facility Abeli. The theoretical framework of the thesis consists of type 2 diabetes, risk factors, prevention, the change of living habits, guidance of the elderly and supporting the motivation for self treatment. For the treatment of type 2 diabetes it is extremely important to chart risk factors and prevent the development of the disease by influencing the living habits of the elderly. To be able to achieve the best possible results for the elderly, health care professionals must have knowledge and skills to motivate and guide the elderly in their self treatment and to be aware of the special features in the aging process. Producing the folder began from the need of the personnel in the assistant living facility Abeli to get collected information about the treatment of the type 2 diabetes for the elderly. Working together with the personnel of Abeli, the folder was developed such as to serve their needs as well as possible. Meetings with the personnel of Abeli were held in the different stages of the process and material for the folder was collected during this process. Diabetes nurses in Pieksämäki and Mikkeli, the staff of the library and the chief physician Pirjo Ilanne-Parikka were also supporting and helpful in finding proper materials for the folder. The functionality and authenticity of the first version of the folder was evaluated by the diabetes nurse Taru Luukkola in Pieksämäki. The final version of the folder was given to Abeli in November 2011. In the report section of the thesis the developing process of the folder was described and evaluated. During the process of making the folder the cooperation with the personnel of Abeli didn`t work as planned, therefore the role of the personnel remained smaller as was planned. On the other hand, the developing process of the folder has been enriching in many ways and the knowledge that we have received during this process about type 2 diabetes and the treatment of the elderly will be useful in the future.
